Assessment Package

Our Occupational Therapy assessment package begins with a caregiver meeting and assessment to better understand the individual’s strengths, needs, routines, and participation in daily activities. The OT may assess areas such as motor skills, sensory processing, self-regulation, self-care, feeding, visual-motor skills, executive functioning, and play or social participation. Following the assessment, families receive individualized recommendations and goal areas.  Note: a report can be offered at additional cost if requested

Services Delivery:

• Pre-screen with Clinical Intake Coordinator
• Pre-assessment call with Occupational Therapist via phone or virtually
• Observation (in centre) *additional mileage cost associated with in-home session.

Cost: $742.50

Individual Therapy Sessions *requires an assessment package

4-week package

i. 1:1 therapy tailored towards recommendations made during assessment for 4 weeks in centre *additional mileage costs associated with in home sessions

ii. Parent training for recommendations

Cost: $990.00

Note: Additional service packages available based on OT recommendation and need are available and would be determined at the end of the 4 week package.
